An initial private consultation usually takes up to 1 hour.

This time allows for:

Thorough discussion of your medical and lifestyle history.
A comprehensive dietary assessment and evaluation.
Provision of advice tailored to your specific requirements (This professional plan is subject to the normal provisions of copyright).
Your questions answered by Mark.

All consultations are confidential.

Patient consultation fees

Initial standard consultation: $120.

Follow up visits up to 30 minutes duration: $60.

If you have Health Fund "Extras" cover, a rebate may be claimed off fees as Mark has the requisite Provider Number.

For reduced rate service:

For pensioners, Mark can offer a reduction off the relevant full rate service (on request).

Fees under a Medicare Program

Mark is a registered professional Dietitian with the Government's Enhanced Primary Care Program under Medicare.

You do not pay any fees for follow up visits to Mark under this program. You will be charged a gap only for the initial visit. You can have up to 5 visits to Mark as a Dietitian per calendar year.

Therefore we suggest you first check whether your GP is a participating Doctor. If "Yes", book an appointment emphasising requirement is for a Primary Care Plan. If your GP agrees, the relevant Referral Forms (GP Management Plan) to Mr Mark Surdut, 139 Hawthorn Road, North Caulfield, for 5 services would be prepared by the GP. Mark then handles all paperwork with Medicare on your behalf.

Other services

Fees for other services such as Tailored Nutrition and Wellness Workshops can be discussed.

If you have other relevant professional needs, Mark would be pleased to work on a consultative package.

Changing an appointment date

Industry practice requires 24 hours notice of an appointment cancellation please. Otherwise, depending on circumstances, the normal applicable fee may apply.
